All three use controlled heat to encourage your skin to build new collagen and elastin over time. Collagen and elastin are the proteins that keep skin firm and springy. As we age, we make less of them, and the skin on the cheeks, jawline and neck can start to look less firm. According to the Cleveland Clinic, energy based treatments work by warming the deeper layers of the skin so the body responds by producing more of these proteins.
That is where the similarities end. The three treatments differ in the kind of energy they use, how that energy gets into the skin, and what that means for needles, numbing and downtime.
How does XERF work?
XERF is a monopolar radiofrequency treatment made by Cynosure Lutronic and cleared by the FDA. It sends two radiofrequencies (6.78 MHz and 2 MHz) into the skin in a single pulse, so the energy reaches the shallow, middle and deep layers at the same time. The handpiece glides over the skin. There are no needles.
A built-in cooling system keeps the surface of the skin comfortable while the energy works underneath. That is why XERF does not require numbing cream. Most people describe the session as warm, not painful. A face and neck session takes about 45 minutes, and you can go right back to your day. Mild redness, if it appears, usually fades quickly.
Plans are typically two to four sessions spaced about a month apart. Skin often feels tighter right away, and the look of firmness builds gradually over the following weeks as collagen rebuilds. Individual results vary.
How does Morpheus8 work?
Morpheus8 is a radiofrequency microneedling treatment. Instead of gliding over the skin, a handpiece with a grid of fine needles enters the skin and delivers radiofrequency energy at the tip of each needle. The idea is to combine the collagen response of microneedling with the heating effect of radiofrequency.
Because needles are involved, Morpheus8 sessions are usually done with numbing cream applied beforehand, and some providers offer additional comfort measures. The Cleveland Clinic notes that radiofrequency microneedling commonly causes redness, swelling and pinpoint marks for a few days after treatment. Many people plan a few quiet days afterward. How does Ultherapy work?
Ultherapy uses focused ultrasound rather than radiofrequency. The device sends ultrasound energy to specific depths under the skin, including the layer that surgeons work on during a facelift, without cutting the surface. It is FDA cleared to lift the skin on the brow, under the chin and on the neck, and to improve the look of lines and wrinkles on the chest.
There are no needles, but Ultherapy is often described as more intense than gliding radiofrequency treatments because the energy is delivered in focused points. Many providers use pain medication or numbing beforehand. The American Academy of Dermatology explains that with ultrasound tightening, most people see modest lifting within two to six months after one treatment, and some choose more than one session.
Which treatment hurts the least?
XERF is usually the most comfortable of the three because it uses integrated cooling and does not break the skin. Morpheus8 involves needles and normally requires numbing. Ultherapy involves no needles, but focused ultrasound can feel sharp or deep, and providers often offer medication beforehand.
